Please call or submit the form below for INITIAL CONSULTATIONS ONLY for any MSK Rehab services (physiotherapy, bracing, massage, chiropractic, and orthotics).  Follow up appointments can be booked via telephone or in person at the appropriate service desks.

MSK Rehab services are not covered by OHIP or ODSP.  DIrect billing to private health insurance, WSIB, or automobile insurance is available.

For Orthopaedic Surgery and Sports Medicine consultations – a referral from a primary care physician (family doctor, walk-in-clinic, etc.) or nurse practitioner is required and should be faxed to the surgeon’s or sports medicine physician’s office directly.
