Please call or submit the form below for INITIAL CONSULTATIONS ONLY for any MSK Rehab services (physiotherapy, bracing, massage, chiropractic, and orthotics). Follow-up appointments can be booked via telephone or in person at the appropriate service desks.
MSK Rehab services are not covered by OHIP or ODSP. Direct billing to private health insurance, WSIB, or automobile insurance is available.
For Orthopaedic Surgery and Sports Medicine consultations – a referral from a primary care physician (family doctor, walk-in clinic, etc.) or nurse practitioner is required and should be faxed to the surgeon’s or sports medicine physician’s office directly.
PLEASE NOTE: For all Orthopaedic surgery inquiries, please directly contact the office of your Orthopaedic Surgeon. Regulations require that all referrals to Orthopaedic Surgeons must come from your Primary Care Physician or Nurse Practitioner.
